Just write down the time, date, and pressure every time you check it – why do you need a fancy chart?

keeping track on your own is a good start, think your doc can give u a chart. there is also lots of sites out there that give you lots of help in lowering your blood pressure. a few years ago my husband had really high blood pressure and the doctor told him to change his eating habits, recommended staying away from certain foods as well as to start eating certain foods. i think this was the site he got his plan and charts from
